Comorbidity definition by USRDS Medical Evidence Report comorbid condition and ICD9 diagnosis code. Comorbidities were assigned either if they were present on the Medical Evidence Report, or if coded in at least one inpatient claim or two outpatient claims at least 1 day apart. Table S2.
